Executive Gold Pricing

 

$395 per month per person 18-64 yr

$425 per month for person 65+ yr

$725 per month for couple 18-64 years of age

$765 per month mixed-age couple (one 18-64; other 65+ years)

$795 per month couple where both are 65+

Family: $1050 per month (two adults + up to two children; each additional child $35 or adolescent at their price).

Executive Gold Family provides the highest level of access, diagnostics, and physician availability for households with complex or high-acuity needs. Family membership provides access for acute care visits for children 4-11. Ongoing primary pediatric care, developmental assessments, behavioral evaluations, and routine childhood immunizations are not included. As distinguished from per patient pediatric care, Executive Gold Family membership is designed for households who value rapid access, visit and diagnostic complexity, and physician availability for same day evaluations, extended care hours, an in-office pharmacy, an in office complex moderate complex laboratory distinguishing true need for antibiotics, and experienced acute care for their children: we complement a child’s primary pediatric care.

Membership Commitment

 

To provide stable, thoughtful, relationship-based care, all memberships begin with a 12-month commitment, similar to how most Direct Primary Care practices operate.

After your first year, membership continues month-to-month with 30-day notice.

No long-term contracts. No penalties. Just continuity.
